All pets admitted for boarding are required to be up to date on all mandatory vaccinations and free of fleas and ticks. If these requirements are not met, appropriate treatment will be administered upon admission at the owner’s expense.

If medications are necessary for treatment or handling, I authorize Akron Medina Veterinary Hospital & Pet Resort to administer such medications.

I acknowledge that I fully understand the check-in and check-out procedures. I understand that billing begins on the day my pet is checked in to the resort.
